National Exit Test • NExT 2027

One Examination.
One Medical Pathway. |

A premium AR-MAC’S guide to the National Exit Test — understanding its proposed two-step framework, eligibility, clinical competency focus, current implementation status and how MBBS students and Foreign Medical Graduates can plan ahead.

Current NMC position

NOT OPERATIONAL

NMC’s March 2026 FAQ states that NExT is not operational at present.

Framework

tep 1 + Step 2

Purpose

Licensure + PG

Regulation

NExT Regulations 2023

Important 2027 status notice

Do not present a 2027 NExT exam date as confirmed. NMC's current FAQ says NExT is not operational, while its 13 July 2023 public notice deferred the examination until further directions. The schedule, registration window and implementation date must be confirmed through future NMC notifications.

01 / OVERVIEW

The NExT Framework.

NExT is designed as a common qualifying/licentiate examination and postgraduate entrance mechanism under the NMC framework. Its final operational implementation and applicable transition rules must be followed through official notifications.

LICENSURE

Permission to Practise

The NExT framework is intended to certify eligibility for registration to practise modern medicine in India.

POSTGRADUATE

PG Entrance

When operational, NExT is intended to serve as an entrance examination for postgraduate medical education as prescribed by NMC.

COMMON FRAMEWORK

IMG + FMG

The NMC framework describes applicability to Indian MBBS students and Foreign Medical Graduates subject to the applicable rules.

02 / Two-Step Architecture

Knowledge → Clinical competence.

The NExT Regulations 2023 outline two separate examination steps with different purposes and assessment styles.

NExT Step 1

Theoretical Examination

Designed as a computer-based theoretical examination using multiple-choice type questions.

NExT Step 2

Clinical / Practical Assessment

Designed to assess clinical skills, practical knowledge and patient-management competence.

03 / Proposed Step 1 Pattern

A six-paper architecture.

The detailed paper distribution below reflects the NMC-published proposed framework. It should not be treated as a confirmed 2027 operational schedule.

The question counts and timings shown are from the proposed/draft framework supplied in the source material and NMC regulatory framework; they are not a confirmed 2027 exam notice.

04 / Syllabus Architecture

Think in systems, not subjects.

NExT preparation should connect foundational science to clinical reasoning, diagnosis, treatment and patient management.

PRE-CLINICAL

Foundational Sciences

  • Anatomy
  • Physiology
  • Biochemistry

PARA-CLINICAL

Applied Foundations

  • Pathology
  • Pharmacology
  • Microbiology
  • Forensic Medicine

CLINICAL

Patient-Centred Medicine

  • General Medicine
  • General Surgery
  • Pediatrics
  • Orthopedics
  • Dermatology
  • Psychiatry
  • Obstetrics & Gynaecology

05 / Preparation Strategy

Build NExT readiness early.

Because the operational date is not currently confirmed, students should focus on durable clinical reasoning and MBBS curriculum mastery rather than relying on speculative calendars.

01 / FOUNDATION

Core Concepts

Strengthen anatomy, physiology, biochemistry, pathology, pharmacology and microbiology.

02 / INTEGRATION

Clinical Links

Connect mechanisms to symptoms, investigations, diagnosis and treatment.

03 / APPLICATION

Case Reasoning

Practise clinical vignettes, differential diagnosis and patient-management decisions.

04 / SIMULATION

Exam Readiness

When an official pattern is notified, transition into timed mocks and the prescribed assessment format.

06 / IMG & FMG Planning

Two routes. One regulatory destination.

The applicable pathway depends on the candidate’s status, medical qualification, internship and regulations in force at the time of application.

Registration / PG Pathway

Foreign Medical Graduates should not assume that a future NExT implementation automatically changes every current FMGL requirement. Verify the regulations applicable to your batch and qualification.

07 / Official Status

What is confirmed — and what is not.

AR-MAC’S separates published regulation from future implementation so students do not plan around rumours.

CONFIRMED FRAMEWORK

NExT Regulations 2023

NMC lists the National Exit Test Regulations, 2023 dated 27 June 2023 on its rules and regulations page.

CURRENT STATUS

Not Operational

NMC’s March 2026 FAQ explicitly states that NExT is not operational at present.

2027 DATE

Not Confirmed Here

No official 2027 NExT examination date should be published until NMC issues an applicable notification or schedule.

FAQs

Frequently asked questions

Do not assume that it is. NMC’s March 2026 FAQ states that NExT is not operational at present. A future official notification is required for implementation details.

NMC’s FAQ describes NExT as intended to replace existing examinations including the Screening Test and NEET-PG (or relevant entrance/qualifying examinations) when NExT is implemented, subject to applicable regulations and transition arrangements.

No. It is part of the proposed framework. Students should not treat it as a confirmed 2027 operational paper schedule unless NMC publishes the applicable final notification.

The NMC framework describes applicability to undergraduate medical students in approved medical colleges and Foreign Medical Graduates approved for the relevant purpose, along with other categories that NMC may specify.

Build strong MBBS curriculum knowledge, clinical reasoning and patient-management understanding. Follow NMC notices rather than relying on social-media dates or unofficial exam announcements.

Track the NMC Foreign Medical Graduate Licentiate Regulations and FAQs applicable to your qualification and batch, and do not assume a future NExT transition automatically removes or changes existing requirements.

Prepare for the pathway. Not the rumour.
